User Defined Fields
User Field: The name of the question to ask the user
Type: This is the type of answer field. You may choose from Lists, text, numbers, dates, multi-lists (for lists that will contain more than one answer), etc.
Min Value: A defined minimum numeric value for the answer
Max Value: A defined maximum numeric parameter for the answer
Min Length: A defined minimum character length for the answer
Max Length: A defined maximum character length for the answer
