Attribute: Enter the name of the attribute.
Group: The group the attribute belong in. This will separate the attributes when looking at them in the Edit Attributes window.
Form Name: The form name associated with this attribute. Attributes with a form name will display another screen when selected.
Description: The description of the attribute. This description will appear in the Attributes window.
