Account #: The code of the Organization the phone calls were made to.
Client: The name of the Organization the phone calls were made to. Organizations may be added/edited/deleted in the Organization Screen.
Start Date: The start date for the report.
End Date: The end date for the report.
Extension: The extension the phone calls were made from/to.
